求助，申请Consent to let的信怎么写才正式？

打电话问了汇丰mortgage advisor, 对方让我正式写封信要求consent to let，他们帮我转交或log in online banking 发过去…
请问各位大神有没有模板啊，我英文不溜，网上找了一圈不懂怎么写才“正式” {:5_145:}

我要求的原因是家离小孩学校太远，需要搬到近点的地方方便接送，所以打算出租目前的房子。

consent to let request

Dear sir/ madam
Good morning. I hope you are well. I am writing to seek your consent to let my property for 多久。
My personal details are as below

银行肯定还会跟你要
你的收入证明，
你目前房屋证明，
还有预期的出租租金证明，注册的中介可以出证明。
你的银行存款证明。如果rent void，怎么支付贷款。
你都现在准备一下吧。可以一起寄过去。

原来还需要那么多文件啊，我看其他论坛有人分享说打个电话要求，两天就收到consent，还以为那么简单

要出租租金证明还需要找中介来survey了

那可以等银行跟你要，你再提供也行的啊。先把你手里有的材料给出去
