本人曾有一跟母姓的别名，有些人会用它叫我。但从未改过现在正式名字，包括身份证或护照。初签时的申请表也填上了。当时好像也不需要更多解释说明或提供证据。
现在续签的表要填日期和提供证据，这些我都不清楚。
请问高人，我该怎么填这表？
