准备要递签Tier 4 dependant, 但是遇到两个问题

现在UKBA网站改版,我在apply online的表格里面找不到Tier 4 dependant的表格.
Tier 4 student的表格虽然上面写着dependant也是用同一个表格,但是点进去填资料的时候发现只有general student或者child student的选项,没有dependant可选
另外有一个专门dependant的表格,但是只对应Tier 1,Tier 2,Tier 5,我需要的Tier 4不在其中.请问Tier 4 dependant的online application表格在哪里

关于dependant材料:
我现在有和老婆的joint account statement, council tax, utility bills,以及英国的marriage certificate,这些都是dependant guidance里面建议的材料
我想问一下, 我这次签dependant是单独签,老婆的学生签已经签完,我这次递交申请的时候还需要提供我老婆学校的相关材料,比如她的CAS letter什么的吗
