现在正在填citizenship 申请表，请教有经验的MM下面这些问题应该怎么回答好呢：

问题1.5是要求填SURNAME的，那这个空是不是填First Name 呢？
1.36 Date and place of marriage/civil partnership
这是需要填详细的结婚登记地址吗，比如说市council register office，要不要写邮编呢？还是简单地写结婚的城市（如London）就可以了呢？
3.5 Address of tax office or tax office reference
我现在是全职工作，但是怎么知道tax office的地址呢？
