申请表的第二页的 文件需要哪些？
Document Checklist Check you have the following documents in your envelope before sending it to us:
Page of
Completed declaration form
Two recent identical photographs
Your full birth or adoption certificate
Any other uncancelled non-British passports relating to you
Evidence of one of your parents’ claim to British nationality. Provide one of the following:
• the parent’s full birth certificate or
• the parent’s certificate of registration or naturalisation
Evidence of one parent’s immigration status at the time of your birth
• the parent’s passport at the time of your birth
Evidence of grandparents’ claim to British nationality by providing their birth certificates and, in the case of
grandfathers, their marriage certificates where available. This does not apply if your parent’s British nationality
is based on registration, naturalisation or their immigration status.
Change of name evidence. If the name on the documentation you provide is different to the name you want to
have shown on your passport, provide proof of every change of name. Provide at least one of the following for
each change:
• Marriage certificate
• Civil Partnership certificate
• Gender Recognition certificate
• Enrolled deed poll
• Change of name deed
• Certificate of naturalisation or registration
• Statutory Declaration or Affidavit
• Birth Certificate (upon re-registration)
• Certificate from the Court of the Lord Lyon of Scotland
• Form PD2 for those about to marry or form a civil partnership
If you are reverting back to your maiden or unmarried name, all of the following are required:
• Your birth certificate and
• A decree absolute showing both names and
• A marriage certificate showing both names and
• A signed statement saying that you now use your maiden name for all purposes
If changing the spelling of your name slightly (e.g. Bryan to Brian), changing the order in which your forenames
appear in your passport or dropping a forename, one of the following is also accepted:
• Letter from a council or government department
• Driving Licence
• Bank Statement
• Baptismal/Confirmation Certificate.
