请教关于入籍申请表的两个问题！！

大家好，请问下大家都是怎么填写申请表3.5的问题？
3.5 Address of tax office or tax office reference

还有一个问题，Evidence of lawful residence during the 5 years before the date of the applicaiton…
Your passports OR (护照会交上去）
Letters from employers, educational establishments or other Government Departments indicating presence in UK. (请问大家都有交这些东西上去吗？这些东西交上去加上护照，会不会画蛇添足呢？打算叫现在的employer出信。请问大家有交些例如银行帐单，电话bill,水电bill之类的吗？

是说OR,但是不知道交不交好，之前看到好像有人说交了bill单证明地址，但是看表格的时候又没说要。所以上来求证下。

完全之计就是都交了得了，用不到又不损失什么，省的到时候麻烦
